Community Outreach & First-time Attendees* Deadline March 27th

The Alaska Mental Health Trust Beneficiary Project Initiative is pleased to offer a scholarship opportunity for mental health consumers to attend the 2009 Alaska Mental Health Recovery Education Conference.  We encourage you to apply! We must recieve your application by March 27.  Please write legibly.   You may send your application by mail or fax or submit it online.  If you are a group or family, please fill one out for each person.  You may send the applications in one packet. We must receive your application by March 27th.  Do not wait. Submit or send in your application as soon as possible to be considered.  We will notify all awardees during the first week of April. 

*Full Registration includes May 11 & 12 conference sessions, workshops, breakfast and lunch.

*Air Travel is round trip airfare. Once reservations are made they cannot be changed.

*Lodging is for Sheraton Anchorage. Check-in is May 10. Check-out is May 12. Based on double occupancy.  You will have one roommate.

*Gas Reimbursement is based on mileage to and from your home and Sheraton Anchorage.

*Per Diem will cover one dinner on May 11 and May 12 because breakfast and lunch are provided at the conference.  Due to limited air service in remote areas affecting travel departure times, dinner per diem for May 10 may be provided for early arrivals.

*Shuttle to and from Airport is provided by conference team. Cab fare is approximately $20 one way per person.

Spending Money: We recommend about $20 per day to cover additional expenses for snacks, beverages or personal items. 

You or someone you know may want to consider funding or partial funding.  Examples are friends, family, employers, committees you belong to, provider agencies, civic, service or religious organizations such as Kiwanis, Rotary, Lions, or your church or place of worship.
